What do members need to enroll?
Medicaid members will only be asked to enter the Personal Identifiable Information (PII) and do not need any formal documentation. The Lifeline for Medicaid Members application will ask:
- Member first and last name
- Member Date of birth
- Last four digits of member’s social security number
- Member Residence Address
- Email Address
Information must match the member’s information in their Medicaid file within the Medicaid database, even if out-to-date. (i.e. If a member has moved but has not yet updated their residence, they must enter the residence address on file.)
How do I confirm my eligibility?
All Medicaid members are eligible for the Lifeline Program and Affordable Connectivity Program. We recommend members confirm that they are in the National Verifier BEFORE submitted their application. Members should visit https://www.checklifeline.org/lifeline to confirm that they are listed in the database and the personal information in the National Verifier.
Members WILL NOT be automatically approved unless their information is listed in the National Verifier OR if the information on the application and within the database do not match.
How long does the process take?
Applying is simple and takes no more than a few minutes. Phones will be shipped to the recipient’s home in 1-2 business days.
If multiple members in the same household qualify for Lifeline and ACP, can they receive multiple free smartphones and plans?
No, Federal rules only permit one Lifeline and ACP benefit per household. If eligible members within the same household are married, they are considered one household and do not qualify individually. However, it two eligible members live together but maintain separate finances (i.e. contribute to the bill separately), those two individuals are considered different economic units and qualify separately. Customer attestation is required to confirm they are different economic units.
What if a member is already receiving a Lifeline and ACP benefit?
Individuals currently receiving the Lifeline and ACP benefit can elect to transfer their benefit to this free, new plan. To do so, members simply need to enroll via this application and their prior benefit will be terminated. It is important to ask members if they already receive a Lifeline and ACP benefits, such as for their home phone line. If members enroll in this new plan, they will lose their former benefit and be charged the full amount of the broadband service. One can check the member’s phone bill to confirm if a member already receives a Lifeline benefit.
Do members need to do anything after they submit their application?

Once the member enters their zip code, they will be shown the relevant contact information for our customer support team.
What if a member’s shipping address is different from their residence address within their Medicaid file?
Members can opt to select a separate shipping address. Members must check the relevant box and enter their current shipping address.
Can I enroll on behalf of a child?
Children who are on Medicaid also qualify for the Lifeline and ACP Programs. A child’s guardian can enroll in the program on behalf of that child.
What if I have questions about using my device?
Please contact the wireless carrier with any technical questions regarding using the device.
What if I want to keep my current phone number?
The Member should notify customer support of their desire to port over an existing number. The Member will then receive a text message asking him/her/they to confirm. There will be a few additional steps sent via text message from the Wireless Carrier. The benefit recipient will receive a final message asking to confirm with “Ready”; at that point in time the number is ported/changed.
What if my phone is lost, stolen or damaged?
The Member should contact customer support at the phone number included in the shipping box to request a new phone if their phone has been lost, stolen or damaged. (If the Member no longer has the phone number, please email email@example.com to be directed to the appropriate representative.) The customer service representatives will then go through the checks to prevent waste, fraud and abuse and determine if/when a new phone may be sent.

Willfully making a false statement in order to obtain a LifeLine service beneﬁt can be punishable by ﬁne, imprisonment or disbarment from the program. If the mobile handset is the only phone in your home, residents will not have access to call 911 when the handset is removed for that location. Affordable Connectivity Program (ACP): The ACP is a government benefit program operated by the Federal Communications Commission (FCC) that provides discounts on monthly broadband Internet access service and certain connected devices. The ACP monthly service and device discounts are non-transferrable. An eligible household is limited to one monthly service discount and one device discount. Only one per household is eligible for the ACP program. Eligibility for the ACP will be determined by the National Verifier, administered by the Universal Service Administrative Company (USAC). ACP discounts can be applied to any available Access Wireless service plans. Our standard Terms and Conditions, including the Acceptable Use Policy, apply. ACP benefits may be obtained from any participating provider of your choosing and your monthly service benefit may be transferred to another provider at any time.
Access Wireless will provide eligible households with unlimited voice and unlimited data. Unlimited data includes a fixed amount at high speed. “High Speed” means maximum available network speeds. After monthly allotted high speed data, remaining data provided at 256 kbps. Video typically streams in SD (480p). Lower speeds may affect audio and video streaming, access to certain websites and content, or use of available applications. Offers may vary by state and service may not be available in all areas. Oklahoma residents: SafetyNet Wireless currently offers Unlimited talk, text, and data plans with between 1GB to 10GB of High-Speed data. Unlimited data is offered at 3G speeds, and data speeds may vary depending on device and signal strength. Customers participating in the government Lifeline and/or Affordable Connectivity Programs may receive free service after discounts are applied.
